Go to https://1link.travelsafe.pr.gov/ to fill out a travel declaration and upload a copy of your vax card or your PCR or antigen test result. You will get a QR code which you will need to exit the airport.

I have made this trip several times over the past ~10 years as a non-citizen, and know several other people that have as well (US citizens and greencard holders).
