Southwest Airlines Flight 1380 was a Boeing 737-700 that experienced a contained engine failure in the left CFM International CFM56 engine after departing from New York–LaGuardia Airport en route to Dallas Love Field on April 17, 2018.
As I read it, the shrapnel hit the window and broke it, causing depressurization and the woman was sucked into the hole. The pilot rapidly lost altitude to get down where oxygen was not needed by the cabin, but they still couldn't pull her out of the hole because of her injuries.

The flight had departed from LaGuardia Airport, Queens , New York, about 30 minutes earlier .
