Passenger Locator Form (dPLF) for Italy is an online health declaration that must be completed by every adult traveler wishing to enter Italy. The Italian dPLF is part of the European Digital Passenger Locator Form, along with Malta and Slovenia.

The obligation will be in force starting from May 24, 2021 for anyone entering Italy from abroad for any duration, and by any transport means.
