  3. Passenger Locator Forms (PLFs) are used by public health authorities to facilitate contact tracing in case travellers are exposed to an infectious disease during their travel by airplane or aircraft, ship (cruise/ferry), rail, bus or automobile.

  4. 18 Μαΐ 2021 · Starting from May 24, 2021, anyone who enters Italy (as their final destination) for any length of time from foreign states or territories , on board any means of transport, before entering the national territory, is required to fill in the digital Passenger Locator Form.

  5. 18 Μαΐ 2021 · From May 24, all travellers entering Italy must complete before their departure the Passenger Location Form via EUdPLF App. The digital Passenger Location Form is compulsory for all travellers above the age of 18.   6. Once the form has been sent, passengers receive the dPLF in PDF and as a QR Code at the email address entered during registration, which must be shown directly from a smartphone (in digital format) when boarding. Alternatively, passengers can print a copy of the dPLF to show when boarding.
