Common signs and symptoms are abdominal pain, vomiting, abdominal distension, constipation, fever, diarrhea, tachycardia (pulse >110/min), hypotension (systolic blood pressure <100 mmHg), urine output (<30 mL/h), and tachypnea (respiratory rate >20/min).
Free gas, or pneumoperitoneum, is gas or air trapped within the peritoneal cavity, but outside the lumen of the bowel. Pneumoperitoneum can be due to bowel perforation, or due to insufflation of gas (CO2 or air) during laparoscopy.
As differential diagnoses, a subphrenic abscess, bowel interposed between diaphragm and liver (Chilaiditi syndrome), and linear atelectasis at the base of the lungs can simulate free air under the diaphragm on a chest X-ray.
