How is delayed puberty in boys defined? Boys can start puberty at a wide range of ages, with 95% starting between the ages of 9 and 14, so we consider puberty delayed when it has not started by age 14. The earliest sign of puberty in boys is enlargement of the testicles, followed by growth of the penis and pubic hair.
Hormones, like testosterone, are at their highest levels first thing in the morning. Additional blood testing done at the same time might include FSH, LH, cortisol, ACTH, thyroid hormone studies, growth factors and a prolactin level. We also do genetic evaluations if they are needed.
Boys: No testicular enlargement (≥4ml or 2.5 cm in long diameter) by age 14 years. Girls: No breast development by age 13 years. Boys and girls: may also have delay in adrenarche - Pubic hair, axillary hair, body odor and acne.
