AF Form 988 is a fillable PDF used by U.S. Air Force personnel to request or authorize leave. It ensures that all leave requests are formally documented and authorized. This guide provides detailed instructions on how to fill out and submit the form correctly.
Before departure, you must have an approved leave authorization (AF Form 988, Leave Request/Authorization) or special order and enough funds for expenses, including costs for travel. Do not assume you can return on time by military air transportation.
Users can print an AF 988 when they receive an email notification that the CSS or Base FM has authorized leave and assigned a leave number. Click Profile in the sidebar. On the Open Leave tab, click the View icon to open an authorized leave request.

Create Leave Request. Note: Reservists do not use LeaveWeb to process leaves. Reservists use the AF Form 988 and process leaves manually in DJMS–RC. If they are Active Duty Military Members or Reservists on Active Duty Orders then they have an Active Duty base finance responsible for their Pay and Leave and they fall under that base for LeaveWeb.
Fill out Section 1 of the form according to the example. You’ll find your current leave balance on your most recent LES on myPay. If you need advance leave (using leave before you earn it), your supervisor/CC needs to approve it and you’ll need to fill out Section 2 as well.
