If a name change is wanted for a certificate, please upload a copy of your legal documentation to your APhA Profile and notate on this request form (i.e. court document, not driver’s license). If over a year, there is still a $25 fee.

Where is my reissued certificate? Requests are being processed within 24 - 48 hours. Once request is processed, the reissued certificate will be uploaded to your APhA profile / account and you should receive an email from Education@aphanet.org that it has been uploaded.
The Immunization Quick Reference Guide was designed to enable certified immunizing pharmacists and student pharmacists to stay up-to-date on current vaccinations and guidelines and to help them answer questions from patients and other practitioners.
