BK virus nephropathy is a serious complication of kidney transplantation. Although 10%–30% of kidney recipients have BK viremia, nephropathy occurs in approximately 2% ( 1 ). Nephropathy is most common early after transplant when immunosuppression is at its peak.

Treatment options are limited and if unaddressed, BK nephropathy (BKVN) will progress to allograft failure.
However, among the several types of polyomavirus infectious in humans, since Purighalla et al. first reported in 1995 , BK virus (BKV) has been increasingly recognized as the major pathogenic virus after renal transplantation, and it can lead to BKV associated nephropathy (BKVAN).
In the modern immunosuppression era, BKV nephropathy (BKN) typically occurs in 1% to 10% of kidney transplant patients; 95% of cases are caused by BKV and 5% by human polyomavirus 2 (formerly JC virus or John Cunningham virus) (Gonzalez 2015; Sawinski 2015b; Schwarz 2012).
BK virus-associated nephropathy (BKVAN) is an important cause of graft loss in kidney transplant recipients. The successful advent of increasingly efficacious immunosuppression has been accompanied by high rates of BK viremia (BKV) in up to 30% of kidney transplant recipients [1].
