The Child and Adult Care Food Program (CACFP) is a federal program that provides nutrition training and financial reimbursement for approved meals and snacks served in licensed child care homes, approved relative care/unlicensed homes and approved licensed day care centers.
Child Enrollment Form - This is the informational form that must be kept on file for each child in your care that you claim on the CACFP. This form must be renewed yearly. CIF - Claim Information Form. This form lists the children in your care and must be sent with your Claim each month.

Current Enrollment Forms must be presented to your CACFP Specialist on every home visit along with your CACFP Agreement, Home Application and menus. All records must be maintained for three years plus the current fiscal year, with the last 12 months readily available.
