Serum urea is increased by a high protein diet, in fever, sepsis and after gastrointestinal bleeding. It is decreased in patients with a low protein intake and in liver failure. It is increased during dehydration even when renal function is normal. It is therefore important to consider possible extra-renal factors in interpreting serum urea levels.
