This form must be submitted in person to the Department of Motor Vehicles along with an application for a driver license or identification card in compliance with California Vehicle Code § 12811 (c)(5) and all requirements must be met prior to the issuance of a driver license or identification card with a veteran designation on
Veteran Status and Service-Connected Disability Verification. SECTION 1 - TO BE COMPLETED BY THE APPLICANT REQUESTING VETERAN STATUS AND/OR SERVICE CONNECTED DISABLITY VERIFICATION PRINT USING BLUE OR BLACK INK.
• Visit Any CVSO. Take your DD214 and government identification to any CVSO to obtain your Veteran Status Verification Form. • Visit Any DMV. Go on-line or call any DMV for an appointment. Then bring your Veteran Status Verification Form to DMV. Complete your application; pay any application fees in addition to a $5.00 Veteran designation fee.
Visit a County Veteran Service Office (CVSO) to obtain a Veteran Status Verification Form (VSD-001). (This form is not available from DMV.) Bring the completed VSD-001 to a DMV field office.
12 Νοε 2015 · Interested veterans need to present a valid DD Form 214 to a local CVSO to receive a Veteran Status Verification form. Then, the veteran takes the CVSO verification form to a DMV field office for processing.
Verify Veteran Status: Visit your County Veteran Service Office (CVSO) with your completed REG 17 form and your proof of military service (e.g., DD-214). The CVSO will verify your veteran status and provide the required certification for your application.
Note: You must provide a completed Veteran Status Verification Form (VSD-001) issued by a County Veterans Service Ofice and pay an additional $5 fee at a local DMV ofice to receive the veteran designation on your DL.
