Evaluation forms from several universities are presented to demonstrate the kind of information that might be sought from students and the variety of ways (e.g., end-of-semester, midsemester) and settings (e.g., classroom, laboratory) in which students can be queried.
Students provide feedback on their courses using up to two forms: gathers feedback on students' experience of the course, covering general questions about learning and course organization, and potentially specific learning goals, course elements, and other instructor-designed questions.
It also outlines changes made to Stanford’s end-term course evaluation form and reports to address some of these issues, as well as recommendations for utilizing alternative forms of feedback on teaching effectiveness.
