28 Ιουλ 2021 · The parent or guardian must promptly notify the school of any change of address, phone numbers, and other contact information. Schools may request updated emergency contact forms at least 2 times per year and the parent or guardian must promptly reply to school requests.
Membership Changes The Membership Change Form OSG-1002 should be used to report address changes, transfers, deaths, resignations and name changes. These forms are linked above, or can be found on the member’s only national website. When completed, they should be emailed to organizing@dar.org or faxed to (202) 628-2098. Other Forms of Interest
