Gout is a characteristically intense acute inflammatory reaction that erupts in response to articular deposits of monosodium urate (MSU) crystals. It usually presents with intermittent painful attacks followed by long periods of remission. Most patients with gout have other comorbidities.
The initial attack (flare) of gout is usually monarticular and often involves the 1st metatarsophalangeal joint. Symptoms of gout include acute, severe pain, tenderness, warmth, redness, and swelling. Definite diagnosis requires identification of crystals in synovial fluid.

Acute gouty arthritis presents with a monoarticular red, inflamed, swollen joint, typically in the lower limb and classically affecting the first metatarsophalangeal joint (podagra) 12. It often manifests during sleep, and can later involve more than one joint to become an oligoarthropathy or rarely, a polyarthropathy 12.

1 Απρ 2019 · Results: Four hundred and seventeen articles met inclusion criteria and were included in the review. Articles describing the anatomical pathology of gout in musculoskeletal structures, including bone, tendon and ligaments, synovium and cartilage, were most common.
