Use this form: • If you have premium Part A or Part B, but wish to no longer be enrolled. • If you have Part B, but recently re-joined the workforce with access to employer-sponsored health insurance and wish to voluntarily terminate this coverage.
If you change your mind about cancelling your coverage, you need to contact SSA before the date your coverage ends. Contact your local Social Security office. If you’re dropping Part B and keeping Part A, we’ll send you a new Medicare card showing you have only Part A coverage.

When cancelling Medicare, there are two main options available – disenrollment and Voluntary Termination of Coverage (VTC). Disenrollment occurs when you switch from Original Medicare (Parts A & B) to a private insurance plan altogether. VTC is an option if you want only one Part of your coverage canceled while keeping another part active.
How to disenroll from Medicare Part A. If you pay a premium for Part A and wish to disenroll from Medicare Part A, visit your local Social Security office or by call 1-800-772-1213 (TTY 1-800-325-0778). You will need to fill out a CMS Form 1763 (Request for Termination of Premium Hospital and Medical Insurance).
