Double check the pregnancy settings in CAS for both your sim and the sim they are trying for a baby with. Make sure one can "Become pregnant" and the other can "Get others pregnant". If you still don't see it that means you could be due to a mod conflict.

Today I decided to do an overview of the realistic childbirth mod for the sims 4. I also included instructions on how to download and install the mod!
10 Οκτ 2022 · There is now a new option in the Sims 4 Realistic Childbirth mod for delivering your baby when you call the obstetrician. Pregnant Sims can call their obstetrician in their 2nd trimester and opt for a home birth!
This mod adds realistic childbirth options such as natural delivery and planned C-section. During natural delivery, your sim will arrive at the hospital and their water will break, they will then go into early labor and they can stay at the hospital for up to two days. they will get a total of 3 moodlets during their stay.
