Permanent residents use Form I-90, Application to Replace Permanent Resident Card, to replace or renew a green card. By law, permanent residents age 18 and older must have a valid, unexpired green card (or equivalent identification) at all times.
Lost, Stolen, or Damaged Card – If your Permanent Resident Card (green card) has been lost, stolen, damaged, or destroyed, you must file Form I-90 by mail to request a replacement card.
Anyone who has been issued an expiring or expired Green Card must complete Form I-90 to renew or replace their card. The form is also used by those who have had their Green Card lost, stolen, or damaged beyond recognition.
