In patients receiving posaconazole prophylaxis with the tablet or intravenous preparation, TDM may not be necessary. Consider TDM on a case-by-case basis (e.g. obese patients, concern for breakthrough infection, etc.), as per protocol, or in consultation with ID.
When to consider TDM. Not routinely recommended. Clinical scenarios necessitating TDM should be discussed with ID and/or ASP (e.g. if capsules are opened for administration via feeding tube, concern for breakthrough fungal infection, etc)
The strongest evidence to support TDM of itraconazole is for the prevention of invasive fungal infections in profoundly immunocompromised patients. Many early clinical studies analysing the efficacy of itraconazole were inconclusive, predominantly because they were underpowered.
