  5. After you pass the COT or COMT multiple-choice examination, you will receive a form to register for the computer-simulated practical examination. After the form and fee are received, they are processed within four weeks and you will receive a confirmation notice via email with examination scheduling instructions.

  6. Your sponsor must be an ophthalmologist if you are applying for the COA, COT, COMT, or OSA exam. ROUB and CDOS applicants may have any physician serve as their sponsor. Original signatures are required on the paper application—signature stamps or computerized digitized signatures are not accepted.
