  2. The following information is available on medical and osteopathic physicians with a Kentucky license: The physician profile provides a link to Orders relating to current Board actions taken against a physician's license.

  3. To search an active license/certificate, enter either the last name or license number. Allied Health professionals will require the license number be entered using their identifying letters. (example PAXXXX, SAXXX) You do not need to login for this option.
