The requirements of licensure and practice for Illinois Licensed Social Worker (LSW) and Illinois Licensed Clinical Social Worker (LCSW) licensure are provided by the ACT (225 ILCS 20/) and the RULES in Administrative Code (68 IAC Part 1470). Licensee Application Forms. Applications for Licensed Social Worker (150) and Licensed Clinical Social Worker (149) licenses may now be completed online. Create an account on IDFPR’s online portal, select the applications, and complete and submit them to the Department!

12 Ιαν 2022 · Licensure allows you to legally call yourself a social worker—a title you have earned and are qualified to carry—and allows you additional work opportunities. You may now apply for the LSW license without taking the ASWB exam so long as all other requirements are met.
The State of Illinois Department of Financial and Professional Regulation (IDFPR) requires the following forms when applying for the LCSW examination: Application for Licensure/Examination: 2024 IDFPR State of Illinois Packet (blank application). Write N/A for Part 7 on page 4 of the application.
