Hypotension, arrhythmia, and sudden death have been reported when methylprednisolone doses of 250 mg or greater are administered in less than 30 minutes.

Ratto et al. compared four different doses of methylprednisolone; 160 or 320 mg given orally, or 500 or 1000 mg given IV in four divided doses in adults with acute asthma and found no difference in their forced expiratory volume in 1st second (FEV 1) measurements or length of hospitalization.

Within a 24-h interval, two dose fractions of methylprednisolone produce a stronger and more sustained immunosuppressive response than one single bolus dose.
Results: Methylprednisolone serum concentrations declined in a linear manner in both groups. However, 4 hours after the dose, the mean serum concentration of methylprednisolone was 50.9 +/- 15.1 ng/mL in the elderly group and 37.6 +/- 7.5 (P = 0.07) ng/mL in the young group.
