A perfectly efficient level up is one where three different attributes have exactly ten skill increases each, and the other four have zero; 30 skill increases in a level are possible if you increase 20 miscellaneous skills in addition to the 10 major/minor skills necessary to level up.
"Perfect leveling" is basically just making sure you are getting +5 to 3 skills every time you level up. Ever 2 points a skill from a certain attribute is increased, you get a +1 modifier to that attribute when you level up.

In general, there is very little level scaling in Morrowind. NPC levels and gear are static. In other words, NPCs will have the same items and stay at the same level at level 1 as they would at level 50. if an NPC has iron armor at level 1, they won't be upgraded to ebony armor at level 50.
Your level will affect the types of enemies you encounter, but that's it. So at level 1, you're more likely to face things like Scamps instead of Dremora at Daedric Ruins, but at level 30, that's not necessarily the case.
Leveled Lists are used to spawn random items inside containers, based on the player's level. Simply put, it is a list of items paired with level requirements, which are used for comparison with the player's level. One leveled list always represents either one random item spawned, or none at all.
