Effective February 1, 2022, if the employer would like to obtain a youth employment certificate for a youth, under the age of 18, to begin work, the employer must fill out the Temporary Youth Employment Certificate below.

NC Youth Work Permit. The application for the NC Youth Work Permit is only available via the North Carolina Department of Labor (NCDOL) website. A sample copy of the previous form is available below for reference.
Permitted Occupations (14-15 year olds): Can only work in office or clerical occupations, in retail sales or service occupations including food service and gasoline service stations.
