Initial cyclosporine oral dosing should be twice daily (approximately 5 mg/kg q12h), and therapeutic drug monitoring (either cyclosporine blood concentrations or pharmacodynamic assays) should then be conducted to ensure that the most appropriate dosage is being used for that individual patient.
