Patient Portal. Northwest Health - Porter is pleased to offer patients easy, secure and convenient access to their personal health information via our new MyHealthHome online portal, with access to information for visits.
In the patient portal, you can: View and share your personal health information related to your hospital stays. Have electronic access to your list of medications, diagnoses, allergies, lab test results, patient history and other care information related to your inpatient hospital visit.
Through our new MyHealthHome patient portal, you can: Securely and easily manage your healthcare online; View recent laboratory results; Obtain radiology reports; View current medications; List current allergies; View immunization records; Review clinical summaries of the care you received at Northwest Health
Can I access the portal from my phone? Yes. You can access it from the hospital website, your preferred browser, or download the mobile app MyHealthHome – Oracle Health and sign in just as you would on a computer or tablet.
Since 2019, Porter’s patients have used MyChart, the UVM Health Network’s patient portal, to view their medical information, test results, and request prescription renewals and appointments with their health care team.
