The Individual Support Plan (ISP) provides details about what is most important to an individual with intellectual disabilities so that everyone involved in supporting that individual can focus on those areas.
Fiscal year 2021-2022 ISPs should reflect service type, duration, frequency, and amount that most accurately reflect and will meet the individual’s anticipated needs. ISP teams should reference ODP’s Individual Transition Guide and probe guidance in the annotated ISP to assess risk related to the COVID-19 pandemic.
Foundations of ISP Development is part of a series of trainings on the Individual Support Plan (ISP). This training provides information on the Individual Preferences and Understanding Communication sections of the ISP as well as other aspects of ISP development.
PURPOSE: This Office of Developmental Programs Individual Support Plan (ISP) Signature Form (revised 4/2018) is required to be completed during the following: • Initial ISP meetings
