You must file an Application for Disability Retirement (PSRS-49) to be considered for the benefit. Benefits are subject to approval, denial, or renewal based on your medical documentation and your job description.
Member Forms. To avoid delays or errors, before you complete an application for retirement or disability retirement form, PSERS strongly suggests you attend one of our retirement exit counseling sessions in your area presented by a PSERS retirement representatives.
This information is important and allows an independent medical examiner to make a recommendation of disability or non-disability. SERS retains independent medical examiners to review disability retirement applications and make recommendations to grant either a temporary or permanent disability.
Some forms, such as applications for annuities, must be completed with the assistance of a SERS retirement counselor. Other forms, such as domestic relations orders or powers of attorney, require a third-party to complete, such as a court or a notary.
This pamphlet explains, in brief, the various options available to you as a member of SERS for receiving a monthly retirement benefit payment. It should be read along with relevant information in the current SERS Member Handbook and SERS web site, www.sers.state.pa.us. Review each option carefully.
You, your employer, or someone acting on your behalf with your power of attorney, may file a Disability Benefit Application provided by SERS for disability benefits. Your personal physician also must provide a report stating the basis of your disability.
