The myometrium is the middle layer of the uterus, consisting of smooth muscle, blood vessels, lymphatics, and nerves. The endometrium sits on the luminal side of the myometrium while the perimetrium sits external to the myometrial layer. The myometrial layer is not shed during menses.

Posteriorly, the peritoneum extends between the rectum and the uterus and forms the rectouterine pouch (pouch of Douglas). It is also enveloped in a portion of the broad ligament called the mesometrium.
