Procainamide is dosed in dogs at 2 mg/kg, slow IV bolus, to a maximum cumulative dose of 25 mg/kg over 10–15 minutes, continuing if needed as a constant-rate infusion (CRI) at 25–40 mcg/kg/min or at 10–20 mg/kg, every 6–8 hours, IM or SC.

Procainamide (brand names Pronestyl®, Biocoryl®, Procan®, Procanbid®) is an antiarrhythmic drug used off label (extra label) in dogs and horses to treat abnormal heart rhythms such as atrial fibrillation, ventricular premature complexes, and ventricular tachycardia.

Dosage. Amiodarone. Dog: 8–10 mg/kg, PO, q 12–24 h for 7–10 days, then decrease to 4–6 mg/kg, q 24 h for longterm treatment. Horse: 5 mg/kg/h, IV, for 1 h, then 0.83 mg/kg/h for 23 h, then 1.9 mg/kg/h for 30 h or to effect; or 10 mg/kg, PO, q 24 h. Amlodipine.
In ACLS, procainamide is indicated for the treatment of stable ventricular tachycardia. However, it can also be used in the treatment of AF with RVR including AF caused by WPW. The main side effect of procainamide is hypotension due to decreased inotropy.
Adult Dosage for Procainamide: The use of procainamide is limited in ACLS for cardiac arrest, due to its requirement of slow infusion and its somewhat unknown effectiveness. If used for recurrent ventricular fibrillation and pulseless ventricular tachycardia, 20 mg/min should be given via IV infusion to a total dose of 17 mg/kg.
