Change your address on your Illinois driver's license, state ID card and Vehicle Registration.
Address Change Form. Our office can send you an application to apply to register to vote at your new address. You will have to complete the application and forward it to your local election authority, which will process your application and mail you a voter's registration card.
You must notify the Secretary of State in writing of any change of address within 10 days. Complete this form and return it to any Driver Services Facility or mail the completed form to the address shown on the front. You will NOT receive a new Driver's License or ID card when using this form.

If you need a new or replacement voter-registration card, please send an email with "Need Registration Card" in the subject line. Be sure to include your full name, your Chicago address and your year of birth in your email to: registration@chicagoelections.gov.
Illinois' residents may register online, in person at the office of the election authority, at Driver's license facilities, with deputy registrars who are appointed in each jurisdiction, or via mail using the Illinois Voter Registration Application available in English and Spanish.
If you're moving, submit a permanent change of address request so your USPS ® mail is properly rerouted to your new address. If you're only relocating for 15 days up to 1 year (like going to school or staying with family or friends), submit a temporary change of address request.
