Occiput posterior position. In occiput posterior position (sometimes called sunny-side up), the fetus is head first (vertex presentation) but is facing forward (toward the mother's pubic bone—that is, facing up when the mother lies on her back).
Occiput posterior positions, including direct OP, LOP (Left Occiput Posterior) and ROP (Right Occiput Posterior) are positions favored by certain internal pelvic shapes. This position has some obstetrical significance.
26 Ιουν 2024 · Occiput posterior (OP) position is the most common fetal malposition. It is important because it is associated with labor abnormalities that may lead to adverse maternal and neonatal consequences, particularly forceps- or vacuum-assisted vaginal birth or cesarean birth.
If a fetus is in the occiput posterior position, operative vaginal delivery or cesarean delivery is often required. In breech presentation, the presenting part is a poor dilating wedge, which can cause the head to be trapped during delivery, often compressing the umbilical cord.
Occiput posterior (OP) malposition occurs in 10% to 20% of fetuses at onset of labor, with some citing a range as large as 6% to 41% (Gardberg, Laakkonen, & Salevaara, 1998; Polo, Chang, Herrera, & Pernoll, 2003; Senecal, Xiong, & Fraser, 2005). Its origin has been studied, with conflicting results.
The occipitoposterior (OP) position is the most common malposition. The fetus lies with its back against the mother's, the occiput in the posterior part of the pelvis with the head deflexed. Shoulder presentation is the most serious malpresentation in labour and constitutes an obstetric emergency.
