R65.21 is a specific code for severe sepsis with septic shock, which can cause acute organ dysfunction. It is not the same as sepsis with acute renal failure, which has a different code (A41.9).
Acute kidney injury (AKI) develops in about two thirds of patients with septic shock, 5, 6 and in half of them, AKI develops before presenting to the emergency department. 1 Therefore, it is reasonable to consider AKI as an early sign of sepsis.
