It carries motor innervation to the pharyngeal constrictors, larynx, and muscles of the soft palate (except tensor veli palatini). The spinal accessory nerve is a purely motor entity. It innervates both the trapezius and the sternocleidomastoid muscles.

The accessory nerve provides motor function (movement) to two muscles essential to neck and shoulder movement, the sternocleidomastoid (SCM) and the trapezius, as well as to the larynx (voice box) and other structures in the throat.
The spinal accessory nerve continues alone and heads backwards and downwards. In the neck, the accessory nerve crosses the internal jugular vein around the level of the posterior belly of digastric muscle.
The spinal accessory nerve, considered part of the eleventh cranial nerve, provides motor innervation to sternocleidomastoid and trapezius. A comprehensive literature review and two cadaveric dissections were undertaken. The spinal accessory nerve originates from the spinal accessory nucleus.
