If you are determined ineligible for disability benefits (Social Security Disability Insurance or Supplemental Security Income), you can contact the Social Security Administration (SSA) to request an appeal. According to SSA appeal guidelines, the appeal must be submitted within 60 days of the written notice of the decision.

STRS Ohio sends a notification of pending approval to the member. The school will also receive a copy. This Preboard Approval Letter is addressed to the superintendent or president. It includes the date the Retirement Board will hear the disability case and lists the date the member must stop working if they are still actively working.
STRS Ohio provides several forms that can be completed online and then printed. Forms that are fillable will highlight the fields that are fillable when opened. Many forms require signature(s) and any required signatures will need to be obtained after printing out the completed form.
