Gather the supporting documents needed. Find a Harris Health eligibility center. Wait for your Gold Card to be processed.
New Application. You must provide information about yourself, your household income, your Harris Health System Medical Record Number, and any changes you have had since your last Harris Health System enrollment. Apply Here. Send Documents.
To qualify for financial assistance at the Harris Health, you must be a resident of Harris County. The following co-payments will be requested at the time you receive services and are for Harris Health System services only.
To obtain Harris Health financial assistance you must complete Harris Health’s “Application for Financial Assistance.” Be sure you, your spouse, and ALL children between 18 and 26 years old who live with you sign and date the form.
