Additional purchase permit applicants must fill out the State of New Jersey Application for Firearms Purchaser Identification Card and/or Handgun Purchase Permit, Form STS-033 (Rev. 09/09).
The ORI number is a numerical identifier that is unique to each police department and can be provided by your local police department for your application to be assigned. If you apply to an incorrect ORI, your application will be cancelled.
