MyChart is a online patient portal where you can easily access your UW Medicine medical information in one secure place. With a full-access personal MyChart account, you can: Book and view appointments online; Check-in for your appointment ahead of time with eCheck-In; Pay bills, sign up for paperless statements and request financial assistance
Follow the instructions provided with your activation code to create your MyChart account. Go to MyChart. Enter your: Activation code. Last four digits of your Social Security number. Date of birth. Create a username and password. Confirm your email address (for alerts only).
Please Identify Yourself. All fields are required. Enter your activation code as it appears on your enrollment letter or After Visit Summary®. Your code is not case sensitive. Enter your date of birth in the format shown, using 4 digits for the year.
