VIRGINIA DIABETES MEDICAL MANAGEMENT PLAN. The DMMP is a management tool that is required for the medical care of children with diabetes at school. This form has been adapted from the form developed by the Virginia Diabetes Council Schools Committee. Parts of this form are medical orders.
