6 Ιουν 2019 · All the forms and applications for Registered Nurses and Advanced Practice nurses are all available at the Nurse Portal. This includes Initial applications, Renewal applications, Reinstatement applications and Name/Address Change forms.
Please use the online applications provided and submit any necessary notice via the WVBOP email address. Beginning May 1, 2020 the WVBOP will no longer be printing/mailing permits. Applicants/Licensees will now receive an emailed link to print their own permits.
You may work as an RN in West Virginia, but to work as an APRN you will need to obtain a West Virginia APRN license. You may apply for an APRN license in your Nurse Portal . I hold a West Virginia multistate license and I am also an APRN in West Virginia.
Individuals eligible for Examination for Pharmacist’s Registration must be one of the following: Graduate of an accredited School or College of Pharmacy. Graduate of a Foreign School of Pharmacy (not located in the U.S. or its possessions) who has successfully completed the Foreign Pharmacy Graduate Equivalency Exam (FPGEE)
West Virginia RN license renewal applications are available on the Nurse Portal. The renewal period begins September 1 and any license not renewed by October 31 will lapse and require an additional fee to reactivate.
