When you want to either make a payment to a provider or reimburse yourself using funds in your health and benefit account(s) (FSA, LPFSA, DCFSA, HRA or Commuter Benefits), you will need to file a claim. You can file a claim on the member website, on the MyHealth app, by mail or fax.
To fully settle and release claims of the Settlement Class Members, Bank of America has agreed to make Claim Settlement Payments to the Settlement Class Members and pay for Notice and Administration Costs of the Settlement (the “Settlement Fund”). Defendant will pay $500,000 to the Settlement Fund.
