anemia cases may contain various poikilocytes, including pencil cells (top left) and prekeratocytes (top right, bottom left) [1]. Pencil cells are hypochromic variants of elliptocytes having long axes at least triple the length of the cell’s short axis [2].
Iron Deficiency Anemia: This is the most frequent cause of pencil cells. When the body lacks sufficient iron, a crucial component of hemoglobin, it produces smaller and more elongated red blood cells.

27 Οκτ 2015 · Elliptocytes may be increased in iron deficiency anemia (where they are sometimes referred to as “pencil cells”) and marrow infiltrative processes (with teardrop cells). In hereditary elliptocytosis, caused by a red cell membrane structural defect, at least 25% of red cells are elliptocytes.
14 Αυγ 2023 · The peripheral smear will show small-sized RBCs with pencil cells. The microcytic cells will have a large zone of central pallor and a small peripheral rim of hemoglobin.
Elliptocytes, also known as ovalocytes or cigar cells, are abnormally shaped red blood cells that appear oval or elongated, from slightly egg-shaped to rod or pencil forms. They have normal central pallor with the hemoglobin appearing concentrated at the ends of the elongated cells when viewed through a light microscope .
1 Δεκ 2008 · We observed increased numbers of an infrequently referenced poikilocyte, the prekeratocyte, in iron deficiency anemia (IDA) compared with beta-thalassemia minor and anemia of chronic disease...
