This form is for people who cannot afford to pay court fees for civil cases, such as divorce. It asks for personal and financial information, and requires you to declare under penalty of perjury that the information is true and correct.
If you can’t afford the filing fee, you can ask the court for a “fee waiver” in order to file for free. With a fee waiver, you also won't pay for certified copies, sheriff's service of process fees, court reporter fees, or most other costs related to starting a case.

If you are getting public benefits, are a low-income person, or do not have enough income to pay for household’s basic needs and your court fees, you may use this form to ask the court to waive all or part of your court fees. The court may order you to answer questions about your finances.
