15 Σεπ 2020 · Lung cysts are the most common mimics of lung cavities and present as round parenchymal lucencies with a well-defined thin wall (<2 mm thickness). Emphysema presents as focal areas of low attenuation without visible walls on CT.
The cavity wall varies in thickness depending on the underlying pathology but typically measures at least 2 mm. Cysts, on the other hand, are usually thin-walled (,2 mm). Pathologically, a cavity arises from liquefactive necrosis and subsequent expulsion of debris via the bronchial tree.

It is further characterized by thick walls of greater than 4 mm. 3,4 The differential for these lesions is broad and includes both infectious and non-infectious causes.

Wall thickness <4 mm has been shown to be caused by benign processes while thickness >15 mm are usually malignant. We describe the case of a former smoker in whom an infection was masking a cavitary squamous cell carcinoma of the lung.
