Section 320.0607, Florida Statutes, provides for the replacement of license plates and validation decals when the original license plate or decal has been lost, stolen, defaced, damaged or lost in transit.
License plates, decals, or parking permits that are lost or stolen can be replaced by filling out and submitting an Application for Replacement License Tags and Decals. License plates or decals that are lost will incur a replacement fee.
If you would like to obtain a replacement by mail, submit an Application for Replacement License Plate, Validation Sticker, or Parking Permit (form HSMV 83146). This form can be downloaded here. Proof of insurance (binder, policy or card) from a certified Florida agent or broker is required to purchase and renew a license plate in Florida.
