6 Μαρ 2024 · Hypophosphatemia can be induced by decreased net intestinal absorption, increased urinary phosphate excretion, or acute movement of extracellular phosphate into cells. This topic will review the evaluation and treatment of patients with hypophosphatemia.

The average daily dietary requirement of phosphate is 0.3mmol/kg. Modest dietary restriction of phosphate should not lead to a hypophosphataemic state. However, if the reduction is chronic, or if intestinal absorption is inhibited by phosphate-binders (e.g. calcium or aluminium salts) over a prolonged period, hypophosphataemia may develop.

Typical dose: Phosphate <1.5 mg/dL (<0.48 mM) ==> Initial dose of 30 mM phosphate infused over 4 hours; Phosphate >1.5 mg/dL (<0.48 mM) ==> Initial dose of 15 mM phosphate infused over 2 hours; Repeat electrolytes and provide more as needed.
Adults generally require smaller relative doses than children, between 1 and 2 g of phosphate divided into 2 to 4 doses per day. Gastrointestinal symptoms are the most frequent side effect, which is mitigated by titrating the dose over several weeks.
