The typical clinical presentation of small fibre neuropathy is that of a symmetrical, length-dependent polyneuropathy associated with sensory or autonomic symptoms. More rarely, the clinical presentation is characterised by non-length-dependent, focal, or multifocal symptoms.

Small fiber neuropathy (SFN) is a disorder of the small myelinated Aδ-fibers and unmyelinated C-fibers . SFN might affect small sensory fibers, autonomic fibers or both, resulting in sensory changes, autonomic dysfunction or combined symptoms [ 7 ].
Small fibers have receptors for and release immune or inflammatory signals distally. They contact mast cells and microvessels. Neurogenic inflammation mediated by C-fibers is sometimes visible as flushing and swelling in focal or generalized small-fiber neuropathy (Figure 2A and C).
Small fiber neuropathy has a benign course but can affect quality of life; individualize treatment to control underlying causes and alleviate pain. Small fiber neuropathy (SFN) is common and can be associated with many medical conditions, including reports of an association with COVID-19.
