NOTE: If your conditional permanent resident status (for example: CR1, CR2, CF1, CF2) is expiring within the next 90 days, then do not file this application. (See the What is the Purpose of This Application section of the Form I-90 Instructions for further information.)
Individuals who fall into the following categories must submit Form I-90 by mail: Lost, Stolen, or Damaged Card – If your Permanent Resident Card (green card) has been lost, stolen, damaged, or destroyed, you must file Form I-90 by mail to request a replacement card.
To apply for a Green Card renewal (Form I-90), you must submit a completed Form I-90, Application to Replace Permanent Resident Card, along with the applicable filing fee and evidence of your continued permanent residence.
If you became a conditional permanent resident based on the creation of a new commercial enterprise and a financial investment in the United States, and your conditional permanent resident status is expiring within the next 90 days, file Form I-829, Petition by Entrepreneur to Remove Conditions.
