Infectious mononucleosis is a contagious viral infection usually caused by EBV (human herpesvirus-4, HHV4), a gamma-herpesviridae DNA virus. EBV is implicated in a wide range of human diseases, many of which have mucocutaneous manifestations that can be acute or chronic.

Key Points. Infectious mononucleosis is caused by Epstein-Barr virus (EBV, human herpesvirus type 4) and is characterized by fatigue, fever, pharyngitis, and lymphadenopathy. Fatigue may persist weeks or months. Severe complications, including airway obstruction, splenic rupture, and neurologic syndromes, occasionally occur.
